Project for Assistance in Transition from Homelessness helps people who are homeless and who have a mental health challenge to move forward and leave homelessness behind. HOPES’ PATH Service Facilitator works hand in hand with PATH Participants to help them access mental health services, income, housing, and other critical services that will help make their journey out of homelessness into a more stable and fulfilling life become a reality. Rapid re-housing provides short-term rental assistance and services. The goals are to help people obtain housing quickly, increase self-sufficiency, and stay housed.
